The first thing you’d notice with this site is its exact similarity with www.KmartFeedback.com with the Sears Customer Satisfaction Survey Sweepstake being an exact replica of Kmart Customer Satisfaction Survey (both of which have exactly the same mechanics and grand prize and run by the same survey firm Synovate). This should not be surprising since they both belong to one company called Sears Holdings Corporation which was the result of a merger between Kmart and Sears in 2005.

1. Take the online survey – You will find the survey questions at the sweepstake promotion web site – www.SearsFeedback.com. But first, you will have to make any purchase in any of the Sears store during the promo period because the code you need to input at www.SearsFeedback.com before you can gain access to the survey questions can be found on the cash register receipt issued for your purchase. (It is the sales check number printed on the receipt.) You need to type in the code to have access to the survey questions. You also need to use the code within seven days from the date of the receipt otherwise the code will expire after said period. Take note that the code can only be used once! After you have typed in and submitted the code you will be directed to the survey section where you need to answer all of the survey questions. (Online entries must be completed by 11:59 pm ET on January 28, 2012.)

2. Additional Survey – You may be prompted to answer another set of survey questions in another section for which you will get another entry to the sweepstake. Not everyone will be prompted but those who do get prompted will get an additional chance to win the grand prize.

3. No purchase required mail-in entry –You can still get a chance to win the $4000 Sears gift card without making a purchase. All you need to do is write your name, complete address, your daytime phone number, and your email address on a 3” X 5” card and mail it first class to – Sears Customer Satisfaction Survey Sweepstakes Entries, 90 Rockwood Place, Rochester, NY 14610. You can mail in as many entries as you wish but each entry must be mailed in separate envelopes. (You get one entry per each envelope. Facsimile, mechanical reproductions, reprinted copies will be void! Only hand printed copies will be accepted!)
